Hair loss after a hair transplant does not necessarily mean that the transplant has failed; Dr Viral Desai lists possible causes, shares what to consider ahead.

A hair transplant can be a complete success, and one can still have their hair looking thinner a few years later. The two are not mutually exclusive, as a transplant involves relocating hair follicles, but it does not cure baldness, shared cosmetic surgeon Dr Viral Desai with HT Lifestyle.

Dr Desai explained that during a transplant, hair is taken from the back and sides of the scalp, where the follicles are naturally more resistant to pattern hair loss. As a result, transplanted hair usually continues to grow for many years.

“However, the original hair around the transplanted area can still thin over time, so ongoing hair care and follow-up treatment may be needed to maintain overall density,” he stated. ”The transplanted hairline remains intact, while the natural hair behind it continues to thin over time.”

According to Dr Desai, there are a few common reasons for continued hair loss after a transplant. They are listed as follows.

  1. Androgenetic alopecia: The cosmetic surgeon described this as a lifelong, progressive condition. The surgery restores the hair that has been lost without stopping the hair loss that is still to come.
  2. Operating too early or too aggressively: As Dr Desai explained, “A 23-year-old who wants a teenage hairline may use up his donor hair on the front, leaving nothing for the crown when it thins at 40, and he ends up with an isolated island of hair.”
  3. Over-harvesting the donor area: Dr Desai explained the donor zone of hair is like a limited bank account, and poor extraction depletes it permanently.
  4. Poor graft survival: This is when grafts are handled roughly, kept outside the body too long or implanted at the wrong depth. This is a technique failure, not baldness.
  5. Stopping treatment after surgery: In the words of Dr Desai, “Many patients also stop all medical treatment after surgery, thinking the problem is solved, and their native hair then thins faster.”
  6. Finally, stress, crash dieting, thyroid problems, nutritional deficiencies and illness can cause shedding that is easily mistaken for a failed transplant, cautioned Dr Desai.

Hair loss is a condition that requires management and does not have a one-time fix, shares Dr Desai.

Questions to ask before hair transplant surgery

Dr Desai advised asking the surgeon three questions before going in for a hair transplant:

  1. How will this look in ten years?
  2. How much donor hair will I have left?
  3. What is the plan for my existing hair?

“After surgery, continue the treatment your doctor prescribes and attend regular follow-ups,” noted the cosmetic surgeon. “Hair loss is a condition to be managed, not a one-time fix. The best hair transplant is the one that still looks natural decades later.”
