A special pro forma has been made for the six-day drive which is all set to start from September 10. The pro forma will contain the name, age, residence of every child in zero to five years of age.

In case of resistant cases, the pro forma will also contain the reason why the family had refused to administer polio drop to the child.

The pro forma will be filled by the supervisors, who will also have to ensure that these children had been immunised during the drive only.

The decision to this effect was taken at the polio review meeting at WHO office, Beli Hospital, on Monday.

Polio immunisation drive nodal officer Dr Kalim Akmal said the way the houses were marked during the polio drive had also been changed. Three new marks had been introduced to give more specific information about the child and whether he had been immunised or not.

“The markings are: XR- child not at the residence, XS- child sick, XH- child not in the house but present in the locality, XV- child out of village and will come back after one week and XL- house locked.

For quality work, the number of houses which would be supervised by them, had been reduced from ten to five, he said.
