According to the records of Jindal IVF Centre in Sector 20 here, when Jyoti came to the nursing home on her maiden visit in August, she sought termination of her pregnancy.

However, as per the requirement under the Medical Termination of Pregnancy Act 1971, before termination of pregnancy, signature of a witness is mandatory. So, the nursing home staff asked the person accompanying Jyoti to sign as a witness.

The witness' name is overwritten in records. The witness appears to have first written his name as Ram Kumar and then attempted to change it into Ramesh Kumar.

Dr Umesh Jindal, owner of the centre, said though she didn't remember anything about this particular case, but speaking on the basis of the record with the centre, "the witness' name appears to have been overwritten".

She said that Jyoti told her that she was unmarried and pregnant, so she wanted to terminate her pregnancy. Dr Jindal confirmed that from ultrasound they found that Jyoti was five-week pregnant and her case was fit for medical termination of pregnancy. "We prescribed her medicines but she never turned up after that," said Dr Jindal.
