A 5-member high-level team consisted of Guna CMO Dr NK Sarraf, Civil surgeon Dr AP Agrawal, Dr CD Sharma, Dr Kanti Suman and Dr LN Ojha is inquiring into the case. Rambai Dhakad of Dungasara village, which comes under primary Health Centre, Bhadora, had undergone family planning operation on December 29 and she died on December 31.

Health Department came to know about the incident the same day after the woman was cremated. A high-level inquiry thereafter was ordered.
