Cairns had a recurrence of a hamstring injury during their six-wicket defeat by India in the fifth match of a triangular tournament on Friday.
The New Zealand manager Lindsay Crocker said Cairns had appeared to make a full recovery before the match after passing a fitness test in the nets.
Another Kiwi all-rounder Jacob Oram also has a hamstring problem and is a doubtful starter against India.
