The doctor's encounters with his patients become a window onto the everyday life of the criminal underworld. His efforts start to pay off and, over time, he earns the respect of the prison community. Visits with ailing prisoners become the context for lively and touching personal stories and respect leads to the sharing of greater and greater confidences.

The tale tells the story of one of the largest prisons in Latin America-Carandiru in Sao Paulo, Brazil-and the lives of people within it, ending with a massive prison revolt that took place in 1992.

According to official records, 111 men died on one terrible day in October. Gradually introducing the various prisoners and their lives, the story reveals the mysteries of the prison and makes us understand how this tragic event came to be.
