All these facts have come to light in the detailed inspection report submitted by the joint director (health) Dr KK Chaurasia to the chief medical officer here.
Dr Chaurasia conducted surprise inspection of atleast four New PHCs on March 18 during which he found that most of the doctors and staff were missing from their respective PHC.
But the major irregularity was spotted at the New PHC Haripur Marro. While going through the OPD register he found that entries of three patients were made on March 18. The 'fake' entries of three patients were also made on March 19 and two patients on March 20 in advance.
As a result the JD mentioned in his report that the entries of three and two patients on March 19 and 20 respectively were fake and after going through the attendance register one thing was clear that all the attendance have been filled by a single person.
The report further says, "during inspection at New PHC Daser medical officer incharge Dr PK Dubey and medical officer community health Dr AK Dubey were missing from the PHC. Dr PK Dubey has put advance signatures till March 20 on the attendance register. Non-Medical Assistant PP Yadav was also missing after marking his attendance in advance. Dr Mangala Prasad posted at New PHC Lakshagrah was also found absent from March 16. The pharmacist was found making patients' entries in a plain paper since the OPD register was with the doctor."
"One staff of New Primary Health Centre Katehara, Pushplata Tewari, was also missing after putting her signature in advance till March 20," the report further adds.
