The accident in Yunnan province happened when the truck smashed into the back of the passenger bus, flipping it over and sending it skidding along the highway, Xinhua news agency said.
The report did not say if it was a domestic or foreign tourist bus.
Sixteen people on the bus were killed and two in the truck died, it said.
All 22 injured were hospitalised.
Nearly 73,500 people died in road accidents in China last year, or just over 200 fatalities per day, according to police statistics.
But traffic fatalities in China last year were down by 10 per cent over 2007, as authorities stepped up road safety awareness and improved management of commercial vehicles, the government has said.
