Former chief of BSF E N Rammohan submitted the report to Home Secretary Gopal K Pillai, who handed over it to the Home Minister.

Though the content of the report is not known immediately, official sources said the 69-year-old 1965 batch Assam-Meghalaya cadre IPS officer is believed to have indicated leadership failure during and after the worst-ever Naxalite attack.

Rammohan has also suggested certain measures to mitigate lapses so that such incidents do not occur in the future, the sources said.

The Home Minister will take necessary action after taking stock of the report. He is likely to submit the report to Parliament with an Action Taken Report.

During the fortnight-long inquiry, Rammohan collected relevant information like the command structure, hierarchy and the decision concerning the operation, quality of training imparted to the CRPF men who were attacked by Naxalites on April six killing 75 of them and a state police head constable, and whether they followed the Standard Operating Procedures, the sources said.

He is believed to have analysed call detail records of the mobile telephones used by the deceased and injured personnel of CRPF during the encounter.

During his visit to Tarmetla in Dantewada district and state headquarters Raipur, Rammohan interacted with CRPF officials, local police and civil officials and injured jawans.
