Mayor Abdul Ahl Abbas said a car exploded Wednesday afternoon near a popular restaurant in downtown Tal Afar, a mid-sized city about 90 miles (150 kilometers) east of the Syrian border.
As people rushed to the scene to help, a suicide bomber in the crowd detonated his explosives belt. Abbas said 12 people were wounded.
Police officials expected the death toll to rise.
